Coming home after a stay in hospital is a welcome relief, we all prefer our own home. However, many decisions need to be made and for someone who has lost some of their independence, it can be a difficult process for them or their family.
We regularly support people to come home, either as a rapid response or as a more planned discharge. Either way we understand the importance of getting it right.
We can help with:
- Discharge planning, care planning and advice on changes to the home environment
- Ongoing care and help at home, to assist with the process of orientation and where appropriate regaining precious independence
- Liaising with community professionals, such as district nurses, physiotherapists etc
- Planning the duration of support and linking with families or other support structures
