Support Coordinator- Liverpool

Full Time, plus on call duties.

Salary £18,588 to £21,210

WarrenCare are looking to recruit an experienced Support Coordinator to work within our specialist Support Services. We have provided supported living services since 2003. This is a specialist service tailored for individuals who have learning disabilities, physical disabilities and high dependency needs, such as Autism, visual impairments and Epilepsy. We provide a support service that enables a person to live independently in their own home with twenty four hour support.

Successful candidates will lead a small team of Support Workers within a person-centre practice.

Responsible for establishing clear boundaries and supporting the support work team to offer Service users the scope to explore alternative hopes and aims with new values and a sense of purpose.

You will be required to produce staff rota’s with the willingness to cover absences where required. Maintain a supportive and stimulating environment for Service Users through the provision and oversight of daily routines, activities and social interaction.

You will ensure the registration of Service Users with primary health care agencies e.g. local GP and Dentist. To ensure annual health checks take place and are recorded appropriately and administer medication in accordance with instructions from medical practitioners, keeping records as required and adhering to medication policies.

Ensuring that Service Users have appropriate social activities that encourage social interaction and development. You will also be required to facilitate and participate in such activities wherever possible and appropriate.

Duties will also include maintaining all aspects of written information, making sure they are up to date on files. Completion of daily record book, shift handover book, reconciliation books and any other documentation as required.

  • Previous experience of supporting people in a residential or community based setting is essential. Excellent verbal and written communication skills
  • Ability to multi-task and prioritise own workload to meet deadlines
  • PC literate with the ability to adapt to in house systems
  • Ability to work as part of a team
  • Good organisational skills
